Dear Community Partner,
As someone who lives and/or works in the East Haddam community, you have undoubtedly developed a sense of community life here. The East Haddam Local Prevention Council, a coalition comprised of staff from East Haddam Youth & Family Services, Inc. and community volunteers, is very interested in your perceptions of our town and the implications for the quality of life of our young people.
The East Haddam Local Prevention Council works to create a unified community approach to prevent the use of alcohol and drugs by our community’s young people and to provide them with the opportunities, skills and values they need to grow into healthy, caring and responsible adults. The success of our efforts is, to a considerable degree, affected by prevailing community attitudes regarding underage alcohol and drug use. These attitudes represent the soil in which the seeds of social action and change will either flourish or wither.
We are inviting you to help us benchmark current community perspectives regarding the prevention of youth alcohol and drug use by completing a survey regarding the “readiness” of our community to support actions that will enhance the well-being of our children. We recognize that you are not an ‘expert’ in this field, but it is important that we get the perspectives of a cross-section of people who live and/or work in East Haddam. Your responses to this survey are completely anonymous. You will not be asked your name or other identifying information. Your electronic address (IP address) will be stripped from your responses by the vendor who provides the survey software (SurveyMonkey.com). And results from the survey will be released only in aggregate form in which no individual responses can be identified.
The results of our readiness survey will help us plan the future efforts of the East Haddam Local Prevention Council. We hope you will take a few minutes to share your perceptions of the East Haddam community.
If you have any questions regarding this survey, we would be happy to talk with you. You can contact Toni McCabe, Director of the East Haddam Youth & Family Services, Inc., by telephone at (860) 873-3296 or via email at email@example.com.
